Job Description
ASSIGNMENT DETAILS:
- Coverage dates needed: May 6-8 (8 hr guarantee)
- Schedule: Mon-Thurs 8a-5p lunch break 1p-2p, Fri 8a-3p with no lunch
- Patient volume: 30 patients per day (75% adult,25% pediatrics)
- Required procedures: Combination of hygiene, fillings, extractions, crown preps
- Credentialing timeframe: ASAP
- Mileage and lodging covered
QUALIFICATIONS AND REQUIREMENTS:
- AZ state license Required
- Certifications: BLS
