Job Description
ASSIGNMENT DETAILS:
- Dates of coverage needed:
- Nov 12, 18, 19, 25, 26
- Dec 16, 17, 23, 24, 30, 31
- Hours: 8:30am – 5:00pm (1- hour lunch)
- No call
- Procedures: All General Dentistry – Including stainless steel crowns and pulpotomies for children and extractions
- Root canals for adults preferred but not necessary
- 15% adults, 85% children
- PPD: 25
- Must be comfortable working on children
- Full staffed with dental assistants who complete all X-rays, Coronal polishing, and fluoride applications.
- Credentialing: 30-60 days
QUALIFICATIONS AND REQUIREMENTS:
- Active AZ license required
- Medicaid/Medicare preferred but not required
- BLS
