Job Title: Locum Dentist (Eureka, CA)

Location: Eureka, CA
Rate: $1100/day
Schedule: Monday to Friday, 8 AM - 5 PM
Patient Mix: 50% Kids / 50% Adults
Duration: 3 months minimum, with the option to extend (no exceptions)

Additional Benefits:

Key Responsibilities:

Provide comprehensive dental care to a mixed patient population (children and adults).
Ensure timely chart closure within 72 hours after each patient visit.
Follow all clinic and procedural protocols.

Qualifications:

Active DEA license required before the start date.
Must be willing to commit to a minimum of 3 months.

Credentialing Requirements:

Credentialing process typically takes 30-60 days (six weeks for timely submission of application and documents).
Required immunizations (must be completed prior to start):
2022-2023 Flu Vaccine
TB Status/X-Ray Results (must be within 90 days before start date)
Positive MMR Titer or Complete Immunization Series
Positive Hep B Titer or Complete Immunization Series
Positive Varicella Zoster Titer or Complete Immunization Series
Tdap Immunization (within the last 10 years)
