Schedule: Monday–Thursday, 8:00 AM–5:00 PM; Friday, 8:00 AM–1:00 PM

Coverage: Oct 9. Current active need is Oct 9, 15-16, Dec 3, 14, 23-24, Jan 4-7, 11-14, 18-21, 25-28, Feb 1-4, 8-11, 15-18, 22-25, March 1-4, 8-11, 15-18, 22-25, 29-31, April 1, 5-8, 12-16, 19-23, 26-30, May 3-7, 10-14, 17-217

Practice: Aspen Dental
-
Clinical Support: 3 Dental Assistants and 1 Registered Dental Hygienist

Consistent coverage is preferred
-
Aspen experience is a plus
Clinical Responsibilities
The ideal provider should be comfortable performing a broad range of general dentistry procedures, including:

Simple and surgical extractions
-
Full-mouth extractions
-
Denture procedures and denture steps
-
Digital dentures
-
Crown & bridge
-
Comprehensive treatment planning
Providers should note any procedure limitations at the time of presentation.
Credentialing Requirements

Active Indiana CSR required at the time of submission
-
Active DEA required and must cover all applicable schedules

Providers must meet all credentialing requirements prior to beginning the assignment
